1. Professional Responsibility
All spectacle (glasses) services provided through this website are performed under the supervision of a licensed optometrist registered with the College of Optometrists of Ontario.
2. Patient Assessment
The optometrist will review factors affecting spectacle wear, including visual needs, lifestyle, and working conditions.
This may be completed via:
- Online questionnaire
- Telephone or video consultation
Important: Patients with complex visual needs, atypical facial features, or special conditions may be advised to attend an in-person visit.
3. Prescription Requirements
All prescriptions must:
- Be valid and not expired
- Be issued by an authorized prescriber
- Include prescriber name, address, phone number, and signature
- Identify the patient and include date of issue and expiry
Prescriptions must be submitted in their original, unaltered form (PDF or image).
The optometrist is responsible for verifying the authenticity of all prescriptions.
